New Student Registration- Grades 6-12

New Student Registration for 2019-2020 School Year

Parent/guardians must make arrangements to forward the following documents for registration: Academic record from previous school, Massachusetts transfer card, medical records, discipline file, attendance records, and I.E.P. if applicable. Also, proof of residency and copy of birth certificate are required. Registrations cannot be completed without the aforementioned items.  See links below to print registration forms, transportation form and summer reading information.  All forms are also available in the Guidance Office.

Appointments may be made by contacting the Guidance Department at (508) 866-6144.  New student registration appointments will be scheduled August 20th-August 22nd.

If your child will require bus transportation, please print the Bus Request Form on the transportation webpage and complete and return it to Guidance with completed registration forms:

Destruction of Records- Class of 2012

To the members of Carver High School Class of 2012: In accordance with Massachusetts General Law 603 CMR 23.00, the temporary record of a student shall be destroyed no later than seven years after the student transfers, graduates, or withdraws from the school system. If any student would like to arrange to pick up their records, please contact the Guidance Department at (508) 866-6144 by November 1, 2019. After that time, all temporary records of the Class of 2012 will be destroyed.

Notice of Destruction of Temporary Student Records

Grade 5 Parent Night- June 13, 2017

Parents/guardians of the current fifth graders are invited to attend an informational evening at Carver Middle High School on Tuesday, June 13th at 6:00 p.m.  Tours of the building will be offered from 5:30 to 6:00 p.m. Following the tours, members of the Carver Middle High School administration and school staff members will describe the transition to 6th grade.

Team assignments will be distributed to a parent/guardian in attendance at the conclusion of the presentation. If a parent/guardian is unable to attend, team assignments will be sent home with your child on June 14th.

This is a wonderful opportunity for parents/guardians to ask questions and to participate in their child’s transition to the middle school.

Grade 5 Parent Night
